Summary The purpose of this position is to serve as a Advanced Nurse Practitioner for Pediatric services providing comprehensive direct care to ambulatory pediatric patients at the Inscription House Health Center located in Inscription House, Arizona. Incumbent ensures patients receive quality patient centered care that is consistent with the patient centered medical home principles, and that care is timely, appropriate, cost effective, sensitive and coordinated to achieve desired clinical outcomes. Responsibilities Independently performs highly specialized nursing assessments of considerable difficultly and advanced nature utilizing advanced clinical and analytical skills. Provides comprehensive health maintenance and therapeutic medical care for patients, including health status evaluation and education, independent diagnostic, assessment, preventative care, treatment, care coordination, and follow-up. Conducts comprehensive evaluation, management, and follow-up of chronic and acute conditions in the Ambulatory Care setting, including complex co-morbid conditions like Hypertension, Diabetes, Cardiovascular Disease (CVD), Chronic Kidney Disease (CKD), Chronic Obstructive Pulmonary Disease (COPD), Asthma, Obesity, auto-immune disorders, and Heart Failure (HF) Conducts comprehensive evaluation for pediatric specific acute and chronic conditions, including developmental disorders, growth related, and others. Provides appropriate administrative duties as assigned, including, but not limited to, completion of patient records, peer review, monthly reports, and attendance at required and assigned meetings. Participates in Quality Improvement programs and initiatives, ensuring compliance with AAAHC, Patient-Centered Medical Home, and other applicable standards. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
